Automatic Withdrawal Form

You may use this form to have your payments automatically withdrawn from a financial institution account.

Cancellation Form

You may use this form to cancel your Florida Prepaid College Plan.

Change of Address Form

You may use this form to change the address of the Account Owner, Survivor and/or Beneficiary. The Account Owner must sign in order for the change request to be valid.

Beneficiary Change Form

You may use this form to change the beneficiary (student).

Account Owner Change Form

You may use this form to change the account owner.

Account Owner Change Form (Death)

You may use this form to change the account owner due to the death of the original account owner.

Scholarship Refund Form

You may use this form to request a scholarship refund from the plan.

Cancellation Form (Death or Disability of Beneficiary)

You may use this form to cancel your Florida Prepaid College Plan due to death or disability of the beneficiary.

Survivor Change Form

You may use this form to change the survivor.

Add Local Fee Plan/Dormitory Plan/Tuition Differential Fee Plan Form

You may use this form to add a Florida Prepaid Local Fee Plan, a Florida Prepaid Dormitory Plan, and /or a Florida Prepaid Tuition Differential Fee Plan for the same beneficiary.

Prepaid Plan Application

You may use this form to open a new Florida Prepaid College Plan.

Withdrawal Form

You may use this form to request a withdrawal for qualified higher education expenses; a withdrawal for beneficiary (student) death, disability or scholarship; or for a non-qualified withdrawal.

Automatic Contribution Form

You may use this form to authorize automatic contributions from your financial institution account.

Cancellation Rollover Form

You may use this form to request a cancellation or a rollover from the Florida College Investment Plan.

Account Owner Change Form

You may use this form to change the account owner.

Allocation Change Form

You may use this form to change the allocation of new contributions.

Beneficiary Change Form

You may use this form to change the beneficiary (student).

Change of Address Form

You may use this form to change the address of the Account Owner, Survivor and/or Beneficiary. The Account Owner must sign in order for the change request to be valid.

Allocation Transfer Form

You may use this form to transfer money already invested from one investment option to another. Federal law only allows transfers of money already invested once per calendar year or with a change of beneficiary (student).

Contribution Coupon

You may use this coupon to contribute to your Investment Plan.

Investment Plan Application

You may use this form to open a new Florida College Investment Plan.

Foundation Transfer Form

You may use this form to request using your Foundation Scholarship at an out of state college or private Florida college.

Foundation Annual Progress Report Form

You may use this form if you are a STARS student updating the Prepaid Foundation about your academic progress.
