Materials For Opening an Account

Set Up Recurring (Automatic Withdrawal or Payroll Deduction) Contributions

You may use the Automatic (ACH) Withdrawal Form to authorize automatic contributions from your financial institution account. You may use the Payroll Deduction Form to authorize automatic payroll deductions from a participating employer.

Print a Gift Certificate and Contribution Coupon

Change Investment Contribution and/or Balance Allocations

You may use the Change Allocation of New Contributions Form to change how any future contributions are invested. You may use the Change Allocation of Balance in Plan Form to change how currently invested contributions are invested in the Investment Plan. You may also elect to apply the same allocation percentages to future contributions with this form.

Change of Address for Account Owner, Survivor, and/or Beneficiary

Change Account Owner, Survivor, and/or Beneficiary

Any request to change the account owner, survivor, and/or beneficiary designated on the account must be in writing, signed by the account owner and, for accounts established on or after February 1, 2009, the survivor, notarized, and received by the Board.
