Opt-In to Joint Approval Requirements For contracts established prior to February 1, 2009, the Account Owner and Survivor may elect to subject any future requests for changes of Account Owner, Survivor or Beneficiary, requests for voluntary termination and requests for refunds associated with the involuntary termination to require the approval of both the Account Owner and the Survivor. Opt-In to Beneficiary Right of Survivorship You may use the form below to opt into the right of survivorship for the beneficiary. The form must be signed by you and, for accounts purchased on or after February 1, 2009, the current survivor (if any), and both signatures must be notarized. Once the form is received, the beneficiary will receive a right of survivorship in the event of the account owner’s death if no survivor is designated on the account or if a designated survivor predeceases the account owner. Using Your Benefits
Use Florida Prepaid College Plan Benefits at a Private Florida or an Out-of-State University or College The Prepaid Plan may be used at any eligible educational institution as defined in Section 529 of the Internal Revenue Code. You may use the Transfer Form to request using your Florida Prepaid College Plan at an out-of-state college or private college in Florida. Log In

Summer Dormitory Consent The Dormitory Plan is intended to cover the fall and spring terms but, with signed authorization from the Account Owner, the plan may be used in a summer semester. Using a Dormitory Plan during a summer semester may represent a material reduction in benefits received because, generally, the cost of housing during a summer session is less than the fall and spring. Request a Refund
Request a Scholarship Refund If the beneficiary (student) receives a scholarship, he or she can still use the Florida Prepaid College Plan and, in most cases, use the scholarship to cover other college expenses. It depends upon the type of the scholarship the student receives. The scholarship refund will be based on the value of the beneficiary’s prepaid plan. Request an Unavailable Dormitory Refund If the beneficiary (student) is unable to use the dormitory plan due to insufficient housing space, the account owner may provide proof from the university housing authority of dormitory unavailability on a semester by semester basis and receive a refund.
